Open and read each attached link below and follow the instructions listed in the first document titled "Application Requirements Information", this attachment includes a "Check-Off List" on page-4, with a list of required documents that need to be submitted in person with the Personal History Statement Application, to the Police Academy Office at Otay Mesa HEC, 8100 Gigantic Street, San Diego, CA 92154.  Contact the Police Academy Office to make an appointment to submit your application at (619) 482-6462.

Organize your documents in the order listed on the "Check-Off List", do not staple any of your documents together.
