For all campus use requests for expressive activities, the attached document should be faxed or submitted for approval at the Student Activities Office, Rm. 601C or fax no. 619.482.6493.
The Form is to be completed in full and must be faxed to the Office of Student Activities on the Chula Vista Campus or appropriate office at the Higher Education Centers listed on the form within five (5) working days of the planned Expressive Activity. If request requires amplified sound, equipment or special set up, the request must be submitted at least seven (7) working days of the planned Expressive Activity.
